  • Two Passport Size Photos of the applicant.
  • NOCs: Municipality or Panchayat, Health Department, etc., if applicable.

State or Central FSSAI License Renewal:

  • Duly Completed and Signed Form B.
  • Photo Proof and Address Proof: Proprietor/Partner/Director(s)/Authorized Signatory.
  • Layout Plan: For the food processing plant.
  • List of Directors: With full addresses and contact details, along with their ID and address proofs.
  • List of Food Categories: For manufacturers.
  • Authority Letter: Nominated by the manufacturer to act on their behalf.
  • NOC: From the municipality or any other local body.
  • Equipment Details: Name and number as per installed capacity and horsepower for manufacturing and processing units.
  • Partnership Deed/Affidavit of Proprietorship: In case of a company, a copy of MOA and AOA.
  • Source of Raw Material: Especially for meat and meat processing units, and source of milk-related products and milk collection centers if applicable.
  • Certificate from Ministry of Tourism: For the hotel industry.
  • Pesticide Residue Report: For water used in packaging products like minerals, carbonated beverages, etc.
  • Certificate from Ministry of Commerce: For 100% EOU (in the case of a central license).
  • IEC Registration Documents: Issued by DGFT (in the case of a central license).
  • Supporting Documents: As proof of turnover (for central licenses).
  • Water Analysis Report: For water used in food processing (for central licenses).
  • Possession Proof: Electricity bill, sales deed, etc. (for central licenses).
  • Cancelled Cheque.
